I am much more in agreement with the top 2/3 of your post. The PPIs are overprescribed both in the hospital and in the office, as you point out. Many of these patients take these medicines indefinitely for very questionable indications. The risks of PPIs, however, that you list at the end are entirely unproven. They are weak associations, reported in the literature. The latest breaking story about the PPI-Plavix interaction is already fading. Personally, I think these drugs are extremely safe, but overused.

I did my Master's thesis on the subject of concurrent use of antibiotics and proton pump inhibitors causing increased C-difficile infections. There was a definite correlation between the two. I check for the most common cause of heartburn and reflux in my office with a simple few minute instant result H. Pylori test. You would be surprised just how many have been treated and totally relieved thus stopping their chronic use of antacids. Great article!
